First, please visit our Locations Page to find the clinic you are looking for. Next, click on the location and you will be taken to a separate page. On this page, you will find a button that reads, “Get Directions” which will route you to our clinic. If you still have trouble finding our location, please give us a call at 602-222-2221.
How do I Pay my Bill Online?
You can pay your bill online here. We want to make it easy for you to make your payments. If you have any issues with the online payment platform, please call us at 602-222-2221.
How do I get my Medical Records?
You can request medical records 3 ways. First, you can make a request in the Patient Portal. You can also download the paper request form on our Patient Forms page and fax it to the appropriate office.
Is my Insurance Accepted?
Find out if your insurance is accepted on our Accepted Insurances page. If you do not see your insurance listed, please call us to confirm whether or not your insurance can be taken.
What are your hours?
Our hours differ by location. Please go to the Locations Page to find the clinic you are looking for. Then, click on the clinic you would like to find hours for. It is important to note that our doctors do not regularly operate out of our satellite offices. If you need hours or have questions about your care, please call us directly.
What is your fax number?
Please find our fax numbers by going to the Locations Page to find the clinic you are looking for. Then, click on the clinic you would like to fax and the number will be listed under the phone number.
